How to use your 3mins pre-case reading effectively

So how would you use your 3mins before you embark on the case in your clinical exam?  You are given paper, a pen and a board to write on.

How would you record the "clues"?  Do you have a "structured" format???
How do you remind yourself of what is the "actual question"...eg is it a management case, is it a long case, is it a joint examination etc.

Can you work out the problem list or the differential diagnosis from the clues you are given during this 3mins?

If it is a joint examination station, would you try and "visualize" what you are about to do as a "practice"...or do you just wait for the bell???

Share with us your strategies...
